Patient and Family Centred Care (PFCC) is a patient-centred quality improvement collaborative in which multi-disciplinary teams in healthcare settings come together to work on common problems. It takes place over around 12 months with 3 collaborative learning sessions interspersed by action periods in which improvements in the teams’ services are designed and tested.
The Foundation is an independent charity working to improve patients experience of care and increase support for the staff who work with them. We support over 200 healthcare organisations in all sectors to transform their culture and put the experiences of staff and patients at the heart of their work. We provide training in Schwartz Rounds and patient-centred service design. Through our Heads of Patient Experience (HOPE) network, we offer peer learning and support and the opportunity for members to learn from best practice.
