A Near Miss Incident is an incident where an event or an omission does not develop further to cause actual harm - but did have the realistic potential to do so. Near-misses are free lessons and are as important in terms of the way lessons are learned as those events where actual harm has occurred. Reporting investigation and learning from near miss incidents can reduce the potential for the event to recur in the future. This conference will focus on what can be learnt from outstanding practice in near miss reporting and investigation and how you can Improve the way near misses are investigated in your service. It will look at national developments in patient safety investigation and how you can develop your skills in recognizing the barriers that prevented a near miss from leading to harm.
